Application Forms
A medical certificate or doctor's opinion is required for a KOSMO medical support application; athletes under 14 also submit a legal guardian consent form, and imaging reports, an athlete registration certificate or a health insurance premium statement are added where applicable.
Foundation forms
Form files are being prepared. Please contact us (+82-2-2631-0117) if you need a form.
We recommend applying online. Attach foundation forms in step 4 (document upload) of the online application.

Tips
- Please mask the last digits of any resident registration number before submitting.
- You do not need to include personal information that is not required.
- File formats: PDF, JPG, PNG (up to 10 MB each, 30 MB in total). Make sure text is clearly legible in photos.

Can I upload photos of documents?
Yes. JPG, PNG and PDF files up to 10 MB each are accepted, as long as the text is clearly legible.
