KOSMO medical support follows six steps — online application, document screening, medical review by sports medicine specialists, decision, treatment at a partner hospital, and follow-up — and applicants can track progress with their tracking number.
Application steps
Check eligibility
What you do: Review the criteria and required documents on the eligibility page.

Apply online
What you do: Complete the 5-step form, upload documents, give consent and submit.
What we do: A tracking number (KOSMO-YYMMDD-XXXX) is issued automatically and a confirmation email is sent.
Document screening
What you do: Stay reachable.
What we do: Our office checks the required documents and eligibility, and requests additional documents if needed.
Medical review
What you do: —
What we do: Sports medicine specialists review the medical documents. We email you when the review starts.
Decision
What you do: Check the result and, if approved, consent to sharing information with the partner hospital.
What we do: We decide on support and its scope and notify you by email.
Treatment support
What you do: Receive treatment at the partner hospital.
What we do: We arrange the partner hospital, support treatment costs and notify you when treatment starts.
Follow-up
What you do: Share your rehabilitation progress (optional).
What we do: We record rehabilitation and return to sport and notify you on completion.
If additional documents are requested
When you receive a request email, look up your application on the tracking page with your tracking number and mobile number, then upload the requested documents. The deadline is stated in the request.
Decision and next steps
- Approved: we arrange a partner hospital and ask for your consent to share the necessary information on the tracking page.
- Not approved: we tell you the reason category. Please contact us with any questions.
Privacy
Your information and documents are stored encrypted and used only to review your medical support application. They are securely deleted after the retention period. See our privacy policy.
Frequently asked questions
Who reviews applications?
Our office first checks the documents, then sports medicine specialists review the medical findings.
Will my application be rejected if documents are missing?
No. If we request additional documents, you can upload them on the tracking page.
How do I check the status?
On the tracking page, using your tracking number and the mobile number you entered.
Can I withdraw my application?
